O2 error code locations

Hi guys,
I'm sure this will have been asked before and I'm pretty sure it's here somewhere, but I can't find it!

this is a long shot but I once saw a diagram (can't remember where) that showed the location of all the O2 sensors in the MK3 and which error codes they were likely to throw out. Does anyone have this or if anyone was feeling super kind knock one up for me?

Are all three sensors the same or is the Cat sensor different to the pre cat sensors?

The cat sensor has a longer lead, otherwise the same.  It's 'bank 1 sensor 2 ', not shown in the drawing - MRS models don't have it.
